A Service and Support Administrator (SSA) is a primary point of coordination in the State of Ohio for individuals who have a developmental disability and meet eligibility requirements for county board services. A SSA ensures that the individual's voice, wants and needs are met by working closely with individuals, providers, parents and guardians. eligibility information

In order to qualify for services from the Jackson County Board of DD, you must be a resident of Jackson County, have documentation of a qualifying diagnosis that occurred before age 22, and participate in the completion of the Ohio Eligibility Determination Instrument. Once you are determined eligible, an SSA will meet with you to discuss available services, including Home and Community Based Services (HCBS) waivers and placement on the HCBS waiver waiting lists. transportation

All providers provide needed and necessary transportation to attend the services offered or, for HPC, to run errands and take individuals with DD to doctors appointments. The County Board has our own transportation system, which transports students at Hope Haven and individuals who attend JVAC. Please contact our current Transportation Supervisor Twila Bentley at 286-6491 to change any of your current transportation schedules if your schedule changes as quickly as you can so she can change her routes.  

Family Resources funding

Anyone who has been determined eligible for county board services can apply for this funding. It is designed for those in need of medical equipment, ramps, adaptive equipment, and other services not covered by HCBS waiver funding or for those who are currently on the HCBS waiver waiting lists.

Please call the SSA after-hours phone to report unusual incidents or major unusual incidents that occur outdside of normal hours of 8 am - 4 pm Monday-Friday, or one Holidays and Weekends. If you have a potenbtially life-threatening emergency, always call 911 first.
